By checking this Box and replying electronically to Blue Ridge, I, as a member of the Cooperative, request and agree to receive the following via electronic means: Notice of Member Meetings; Director Election Kits or other background information regarding Resolutions or any matters upon which a Blue Ridge member may vote, including but not limited to biographical information of Director candidates, voting instructions and an electronic link to a proxy; Blue Ridge Annual Reports; Bylaw amendments; and all other communications of the Cooperative with me as a member. I authorize the use of the email address by which I am sending this electronic reply for these purposes. I acknowledge that it is my responsibility to notify Blue Ridge Electric of an alternative email address or to stop electronic communications with me for these purposes and that Blue Ridge will use reasonable efforts to implement the change within a reasonable time period after receipt of notice.
